Diagnosis, screening tests, and treatment for ovarian cancer. Dr. E. Grace Shirley (Consultant Radiation Oncologist, KIMS Trivandrum) explains cancer of the ovaries is usually drowned in the ocean of antacids. Often women have vague symptoms and complaints of abdominal distinction, etc. So, when the imaging is done we see mass in the ovaries. There is no definitive screening test for ovarian cancer but in the case of familial history, women having BRCA-1, BRCA-2 mutations, ovarian cancer patients in women having BRCA-1, BRCA-2, have to go the ovarian ultrasound to rule out malignancy every year. CA-125 is a screening test that can be done in high-risk women and not for women in the general population. Management of ovarian cancer the modality of treatment is surgery. Stage 1 and 2, where the surgeons feel it is operable upfront, in that case, surgeons go ahead with the surgery. The surgical management is entirely different and it should be done by a trained onco surgeon. It has the entire uterus, ovaries as well as the nodes in the abdomen that have to be taken out. Even in stage 3 if the surgeon feels a complete tumor removal can be done, surgery can be proceeded upfront. But if the surgeon feels it's not possible to remove the tumor completely then we have to take a biopsy for confirmation and proceed with neoadjuvant chemotherapy. To shrink the tumor we give 3 cycles, re-asses with CT scan, and if it's operable we proceed with surgery.
